I have this, which page is missing? The best I can tell, my copy is
complete, but I will admit that it has been stored in a box for at least
20 years. BTW, I also have the 2nd volume which has its own oddity. My
copy comprises of signatures printed on what appears to be two grades of
paper. At least they have aged to two different shades of light brown.
Does anybody have a copy in similar condition?

It is not missing, just loose.

I do have books like that but my copy of V2 is not one of them. Huh.
I had assumed if one book in the print run was like that, they all
were.

I'm going to conclude from this observational data that the exact
opposite is true. Instead, presumably this phenomenon happens when,
during a print run, it is necessary to switch over from a shipment of paper
from one supplier to paper from another supplier. So instead of books
all being the same in this respect, books like this are isolated occurences
that happen only a few times for a given title, but which happen to many
titles.
