Why would it be Multiframe if you are digitizing a single image? I would definetely go for Secondary Capture 1.2.840.10008.5.1.4.1.1.7

My question is more like what ConversionType (0008,0064) to use: SI (Scanned Image) or DF (Digitized Film). But this might probably have to go in a separate question.
