Is there something wrong in my configuration?
/root/spam/incoming/spamtest is a test file (posted by me and then
pulled) with msg-id, subject and all the other headers.

Is 'spamtest' a properly formatted message? There must be a header and a
body, even if the body is just one line of anything. There needs to be:

HEADER
<blank line>
BODY

Sometimes, if I just save a header, I simply add:
<blank line>
body

to the file.

I did, but I needed to run as root for it to work. I don't usually run
stuff as root.

Try changing line 43 in nocemlist.php from:
$localeol = PHP_EOL.PHP_EOL;
to:
$localeol = "\n\n";

Also, mske sure there is NO blank line at the start of the file.

There were no such lines, but it is a problem with the file.
