I installed fresh on same server a different domain and a second on different subdomain.

Right away the captcha images don't appear on register.php. Restarting servers and rebooting don't help.

Captcha works on the other rslight installs on the same server. No image appears. No image seems to be generated in the web cache.

What do I need to look at?

Lol. Years ago I used to deliver pizza and a customer had a small trash can
next to her door with a label that said, "Plese place advertisements and
flyers here."

I told her I found it humorous, and she said almost 99% of the crap that
would be left at her door is left in the can.

Another good thing about that is that if spammers harvest addresses and
send mail to that address, they will be listed at uceprotect. I use
their level 1 dnsbl in sendmail and that means those spammers will
rather unlikely be able to deliver their junk to my real inbox.
