Pretty sure it was more the thoracic outlet condition/surgery starting in 2016. It was after that when he was never the same. Before that, he was 25-18 with a 2.53 ERA. After that, he was 25-48 with a 5.92 ERA. In 2015 he had a real good season after recovering from TJ surgery, and played a major role getting them to the World Series. I know he had that bad 9th inning in the last game, but that was as much the manager's fault as his - he was just gassed after pitching a great 8 innings.
