Hello all.

A couple of years ago I was dead proud to have got my blood pressure
down, but the cardiologist thought he'd put me on the treadmill anyway.
He had to abandon the test because he thought I was going to have a heart
attack, as the BP went through the roof.

I was just thinking it may be a tad awkward if I popped my clogs while
out cycling. I go out most days and my usual route includes a 35º incline.

A while back, a local policeman, young, fit as a flea, dropped dead while
cycling.

So, I am thinking of buying a pulse/BP monitor type thingy, for when out
cycling. I know a lot of folks here are gadget freaks, so wonder if
anyone can recommend one.

won't all this anxiety about your BP just raise your BP further?

when you see your BP rising then you will have anxiety about croaking,
raising the BP further

this exercise thing sounds very unhealthy

Not sure you can get a BP monitor that you can use during an activity.

Lots of wearables include heart rate monitoring now, though. I've been
using Garmin devices for years, and Mrs E's just got a low-end one that
she seems pretty happy with. Both provide constant heart rate monitoring
by default, with optional warnings if it unexpectedly goes too high or
too low.

These include a lot more functionality than just heart rate monitoring,
you can probably get something much cheaper if you don't want all the
bells and whistles.

Chest straps will generally be more accurate, but are much more of a
faff than already having the device on your wrist.

Not as such, but I vaguely looked at this area a year or three back,
even used a BP app on my phone for a while and a supermarket-type BP
measure. Both of those were hopelessly innacurate, with consistently
low readings. Ended up getting a proper upper0arm cuff one ilke this

Obviously only for home use, but I concluded that any wearable gadget
would not be able to give accurate BP readings while exercising. Pulse
rate, well yeah, but your body will tell you when that's high anyway -
training freaks use them to keep in a supposedly ideal zone for
whatever they're trying to achieve from a given exercise, not to see
if it's getting too high for safety.

Not quite what you were asking for but this is a really good gadget. It
is not 'wearable' as such but you can attach electrodes or use it simply
in the hands etc. It gives a good display and results can be downloaded
to phone and/or computer where a proper calibrated chart can be printed
for use by health professional.

flippancy in the previous post aside, I have been managing hypertension for
over 20 years. I had a spike in 2021 leading to a referral to a renal
consultant (hypertension comes under renal medicine). All settled down now
but reducing hours and ultimately giving up a stressful job was part of
getting it under control.

From my own experience and what medics tell me, the upper arm cuff type
devices are the only reliable devices to measure BP. I initially tried a
wrist device but it was shit.

I bought an Omron MX3plus[1] some years ago, which is approved for clinical
use. It was not costly. My consultant was very happy to receive a list of
self readings using this device and she discharged me back to my GP some
months ago.

[1] M3 is the current version, �50 in Argos but there are cheaper devices

I'm trying to remember how they took my BP on the treadmill, but I'm not
sure I actually took much notice.

We have an M2 basic. I'm assuming it's accurate, but I wouldn't fancy
trying to strap it onto the bicycle.

FWIW, my BP (controled - losartan) is around 130/80 most of the time and
resting heart rate rarely goes above 60. Lowest I've seen is 48.

even the blurb says it is not accurate compared to clinical devices

my experience of a BP monitor is that any movement will cause the device to
abort the test so not convinced it will work during exercise. Plus you will
not have your wrist in the same place during measurements which is crucial.

I have also read wrist monitors are not good for the elderly with more
rigid arteries.

https://bihsoc.org/bp-monitors/ has some wrist monitors in their approved
list but state they are less reliable as people do not use them correctly.
An upper arm cuff will by design, have the cuff in the right position for
consistent measurements.

Unfortunately I can make a recommendation.

This a rather amazing device which was recommended to me last year (previous Gen). Works with an excellent Smartphone app.
The ECG function and alarms for (low) heart rate and blood pressure have all been in play.

Things have moved on a lot since I got my wrist-worn gadget for £3.50 in
a charity shop - such a lot that I can't remember its make, and more
than a year in hospital and 'respite' care saw its batteries flowing
through its innards, but while it lasted it was within a gnat's crotchet
of the surgery's gold-plated thing, so a good one is worth having.

Operators of hospital ones always worry 'cos my BP is typically 90/45

That's the one - well, the make of the wrist-worn one I got in the
charity shop. Unfortunately the leaked batteries have destroyed it.

Consistency is good - if all you're looking for is an abnormally high
reading and you know *how* inaccurate it is, it'll tell you somehting
useful, surely?

I've had a number of wrist devices, I'd not 100% rely on any of them the current
a band 7 URL:https://consumer.huawei.com/my/wearables/band7/ has been the best
of the bunch by far both technically and VFM, the fatbit the worst. Even so I
can be going for it quite hard, can feel my heart pretty elevated and get
reading of ~100 which is defo wrong not all the time, think it is sweat that
interferes with the functioning.

Clinicians have access to 24 hour strap on monitoring devices, you could discuss
with the Dr. bit of a fag as they fire every 5 minutes or so which will disturb
sleep if used continually the purpose for which they designed.

That's the model I have also bout possibly 10 years ago. After comparing both at
the GP (me) and Cardiologist[1] (Liz) it seems to concur with their devices. I
certainly had white coat syndrome in that it was always significantly worse at
the quacks than at home. Still eventually when my GP concerned about my low
resting pulse asked me to see a Cardiologist I got the pulse is fine, BP OTOH so
now like many I am medicated to control it.

[1] Is also a relative, who she also sees professionally now.

Or maybe not. Unless you do simultaneaous measurements you're not
going to know the accuracy, not whether it's going to be consistently
over or undereading by the same amount.

I bought my arm-cuff one after an incident that the hospital thought
might have been a TIA (it wasn't) and monitored my BP for hours,
trying to get it to drop from a peak of something like 190/120 (I get
very stressed by hospital visits, although post-surgery monitoring has
always been well withing acceptable limits).

The wrist one I'd been using was normally reading 120/70 or
thereabout, so no signs whatsoever of any issue, whereas the cuff one
shows a normal resting BP of around 140/80, so a little on the high
side, something to be aware of. The point was though that I had no
idea that the wrist one was underreading, so it was worse than nothing
and potentially disguising a harmful condition.

That actually looks quite neat, at a decent price, and ticks all the
boxes... but...

This does rather hint at a 'snake oil' factor, which makes me suspicious

No idea. I started taking them after my baby stroke in 2011.

I'm a lot fitter now than I was then, so maybe it wouldn't go up by a
vast amount. Actually, now you mention it, I could look into my records
and see what it was back then. For a laugh.

My two-pennyworth:

First get the diet right…

Get salt to 2.5G/day (difficult, you need to read food labels)(daily
average seems to be about 12g)

Cut red meat to twice a week, say 30g protein at a time (about 150g raw
meat)

Eat more fish, especially oily types

Eat more chicken esp organic

Consider having one vegetarian day a week.

Have lots of fruit

Things to watch for include phosphorus content. This leaches calcium from
the bones, and deposits it in the heart and arteries. This is sub-optimal.
Processed food is rich in phosphorus compounds, which are easily absorbed.
Plant sources are poorly absorbed, about 30%. Average intake is circa
3000mg, get it down to under 800mg. Foods rich in phosphorus include cola
dinks, fizzy drinks, chocolate, ice-cream, beer. Many processed foods have
phosphorus-containing additives, look for the letters ‘phos’ in the
ingredients list.

Then…

Put away the bicycle, get a good pair of walking shoes, and walk a lot. The
Dutch cycle far more than the Brits, yet die from the same major diseases
in the same proportions. Walking is less stressful both physically and
mentally.

Learn Passive Muscle Relaxation. This takes time and effort to master. It
will bring down your BP. It’s how you can come up with acceptable BP
numbers when the doc asks for home monitoring. Ignore the stuff about it
being a therapy, this is a very useful technique that you can use anywhere
(but not while driving). 15 minutes PMR brings my BP down 30 points.

I have a cheap Xiaomi Fitness Band (ha!).

It does heart rate and sleep monitoring. The heart rate function has an
alarm. Disconcertingly the only time this has 'sounded' was when I was
in bed asleep. Not sure what woke me up first - the alarm or the feeling
distinctly weird. Two nights running. Wine had been taken TBF. Might
have cocked up on my medication regime, but not that I recall. Hasn't
happened since.

It measures pulse oxygen too when demanded of it. Seems I am still alive.

BP 'something over something'. Not as high as it got a couple of years
ago, but the medication side effects aren't fun.

I did have a similar thing 'band' that purported to monitor BP but
despite attempting to calibrate it when at the quacks it was shite.

I now have a fancy cuff monitor (Beurer) that can download results to a
PC - except I haven't used it for a while. I really should, in the vain
hope of convincing my GP to reduce the BP meds dose, or try me on
something other than Amlodipine. I think they tried me on a beta-blocker
as first treatment, but IIRC it gave me a cough. ARBs, that I was on for
years have similar side effects to the current stuff, but if anything
worse. I'd actually prefer a cough TBH.

What was Champ saying about the UKRM demographic?

Indeed, but a degree of wine indulgence is a regular weekend evening
meal occurrence without usually such extreme effects. Might have been
something I ate - as in a starter containing unfamiliar matter. Soya -
particularly raw edamame beans is an absolute no-no for me, but the
discomfort then is usually also of the 'high wind' variety.
